My office is located in Lynnwood, WA, near the Alderwood mall. The building is accessible and offers free parking.
Many people enjoy coming to see me in person. I have a long history of providing telehealth, and am happy to see you online, as long as you are in the state of Washington at the time of our visits.

I am not paneled with commercial health insurance carriers, Medicare, or Medicaid. I can provide a billing statement that allows you to seek reimbursement from your commercial carrier. This process is not permitted with government-sponsored insurance. If you are unsure, please call me so we can discuss my fees.
I am paneled with Washington LNI including self-insured workers’ compensation programs. My office manages obtaining your evaluation and/or treatment authorizations as needed.
I am contracted to provide services through the Washington State Division of Vocational Rehabilitation (DVR) and the Washington State Department of Services for the Blind (DSB). Here too, my office will manage obtaining your evaluation and/or treatment authorizations as needed.
